Field Seminar Registration
Advance registration is required. Anyone 16 years
or older is considered an adult. Registration must be received no later than 5 days prior
to the seminar date. Enrollment is limited and applications will be accepted in the order
in which they are received. Upon acceptance,
you will receive confirmation and an information sheet for each seminar in which you are enrolled. Please photocopy this registration
form if you need additional space
Refunds
will be made on cancellations received by our office 5 days prior to the seminar date
(less a $5.00 processing charge.) Seminars not meeting their minimum enrollment 5 days
prior to the seminar date will be canceled. You will be notified immediately and receive a
full refund.
